The Sciatic Nerve is the largest nerve n the body and is formed from nerve roots in the low back and sacrum, specifically L4, L5, S1, S2 and S3 levels. As these nerve roots exit from the lumbar and sacral regions of the lumbo-pelvic spinal regions they come together to form the large sciatic nerve. The sciatic nerve then extends down through your hips, buttocks, pelvis and legs bilaterally, helping our lower extremities to function. When irritation of the sciatic nerve occurs we can experience symptoms of sciatica that show up in the form of numbness, tingling, burning sensations and/or sharp, stabbing or electrical shooting pains.
In most cases, the major symptom of sciatica pain is an intense shooting pain from your lower back or pelvis down into your buttock, the back of your leg (hamstring) and as far down as your calf and/or ankle. Many describe it as a kind of burning sensation that can also be a mild, ongoing ache, or a sharp, electric-shock or jolt. Sciatic pain often gets worse after long periods of sitting, walking or standing and may be exacerbated by coughing, sneezing or other minor movements or activities.
When the pain kicks in, many people will try stretching, resting, heat, ice, pharmaceutical or OTC drugs, steroid injections or maybe even just pushing through it – but it just doesn’t seem to get any better!

WHAT CAUSES SCIATICA PAIN?
For many people suffering with Sciatica pain they frequently have had a history and/or episodes of lower back pain however, there are some who have never had a back pain issue and the sciatic pain seems to ‘come out of nowhere’. Regardless, the pain can quickly become the kind of debilitating pain that radiates down one of your legs or both of your legs and you’re your calves and ankles. The cause of this pain is often related to something that is compressing, impinging or has injured your sciatic nerve directly or the nerve roots that make up the sciatic nerve.

- Spinal Disc Injury: When an injury to one of our spinal discs happens it can cause the ‘jelly’ in the disc to bulge or herniate and ‘pinch’ one the nerve roots that contribute to the sciatic nerve. This can happen at one level or multiple levels of the spine/pelvis.
- Spinal Disc Decay (Arthritis): Decay of the spinal discs and joints of the spine can create biomechanical issues and/or narrowing of the area where the nerve roots exit the spine. This, like a disc injury, can create a scenario where the nerve root gets ‘pinched’, causing sciatic pain.
- Stenosis: The word stenosis means ‘narrowing’ and this narrowing can happen in the Spinal Canal or where the nerve roots exit, called the Intervertebral Foramen (IVF). When the spinal canal or IVF are narrowed they can ‘pinch’ the nerves and cause sciatic nerve pain.
- Spinal Misalignments: When the joints of the spine are even slightly misaligned they ‘compress or pinch’ the nerves that form the sciatic nerve and cause pain.
- Anatomic Leg Length Inequalities (LLI): We find this to be an extremely common condition that is frequently missed by other health care providers. Part of our protocol is to rule in or rule out a LLI when we evaluate for the cause of Sciatic Pain.
- Piriformis syndrome: The large Sciatic Nerve travels underneath the Piriformis Muscle. This muscle is an external rotator of the hip/pelvis and when it becomes tight or spasms, it can ‘pinch’ and irritate the sciatic nerve, causing pain.
- Nerve Injury: There can be many causes of injury to nerves in the body, including the sciatic nerve, such as a mechanical injury or a metabolic injury. It’s vital to know which you are dealing with in order to solve the cause and fix the problem.
Regardless of the cause of Sciatic Pain it’s possible for sciatic nerve pain to cause pain down both of your legs however, for most people suffering with sciatic pain they usually experience symptoms on one side of the body at any given time.
